Elektrikli Scooter Travmasına Bağlı Nefrektomi Yapılan Olguda Anestezi Yönetimi
Özet
Elektrikli scooter (ElS) kullanımı dünya genelinde yaygınlaşmakla birlikte, ElS ilişkili travma sayısında da artış görülmektedir. ElS ilişkili kazalar (ElSK) yüksek enerjili travma olarak değerlendirilmekte ve sonuçları hayatı tehdit edici boyutlara varabilmektedir. ElS kullanımı ile eş zamanlı alkol ve madde kullanımının eşlik ettiği vakalarda ise morbidite ve mortalite daha yüksektir. Bu kazalar sistematik yaklaşım ile değerlendirilip tedavi edilmeli ve tüm sistemler ayrıntılı olarak gözden geçirilmelidir. Cerrahi işlem gereken durumlarda ise mutlaka yeterli ekipman ve kan hazırlığı sağlanmalıdır. Anestezi yönetiminde yakın takip önerilmekte, gross kan kayıplarının eşlik edebileceği unutulmamalıdır. ElSK’da genellikle kas-iskelet, toraks, abdomen ve intrakranial yaralanmalar mevcuttur. ElSK ile ilişkili daha önce bildirilmiş izole böbrek yaralanmasına literatürde rastlanmamış olup, bu olguda ise ElSK sonrası izole böbrek laserasyonu nedeniyle total nefrektomi yapılmıştır. ElSK nedeniyle nadir görülen bu vakanın multidisipliner anestezi ve analjezi yönetimini literatür eşliğinde sizlerle paylaşmak istedik.
As the use of electric scooters (ElS) has become widespread worldwide, the number of ElS related traumas has also increased. ElS related accidents (ElSK) are considered as high energy trauma and the consequences can be life threatening. Morbidity and mortality are higher in cases of concomitant alcohol and substance use. These accidents should be evaluated and treated with a systematic approach and all systems should be reviewed in detail. In cases where surgical procedures are required, adequate equipment and blood preparation must be provided. Close follow up is recommended in anesthesia management and it should be kept in mind that gross blood loss may be associated. Musculoskeletal, thorax, abdomen and intracranial injuries are usually present in ElSK. There is no previously reported isolated renal injury associated with ElSK in the literature, and in this case, total nephrectomy was performed due to isolated renal laceration after ElSK. We wanted to share with you the multidisciplier anesthesia and analgesia management of this rare case with the literature.
