Torasik Endovasküler Aort Onarımı Nedeni ile BOS Drenaj Kateteri Takılan Olguda Epidural Kan Yaması Uygulamasına Rağmen Gelişen Dirençli Baş Ağrısı Yönetimi
Özet
Torasik endovasküler anevrizma onarımı (TEVAR) işlemi torasik aort anevrizmalarının tedavisinde mortalite ve morbidite azalması sağlayan minimal invaziv bir yöntemdir. Anestezi yönetimi dikkatli planlanmalıdır. Endikasyonu olan hastalarda- bizim olgumuzda olduğu gibi- BOS drenaj kateteri yerleştirilmektedir. BOS drenaj kateteri yerleştirilmesi ve/ veya çıkartılması sonrası baş ağrısı görülebilmektedir. Olgumuzda bilimsel literatür gözden geçirilmekte ve epidural kan yaması tedavisi de dahil olmak üzere lomber ponksiyon sonrası baş ağrılarının tanısı ve tedavisinde yer alan güncel öneriler vurgulanmaktadır.
Thoracic endovascular aneurysm repair (TEVAR) procedure is a minimally invasive method that reduces mortality and morbidity in the treatment of thoracic aortic aneurysms. Anesthesia management should be planned carefully. In patients with indications, a CSF drainage catheter is placed - as in our case. Headache may occur after CSF drainage catheter placement and/or removal. This case reviews the scientific literature and highlights current recommendations in the diagnosis and treatment of post-lumbar puncture headaches, including epidural blood patch therapy.
